After reviewing our checklist please complete this form so we may assist you. We recommend submitting this form at least 20 business days prior to the request date. We will confirm the status of your request 3-5 business days following your submission.

This form assists us in determining availability and who might be most appropriate to attend your event if the president can’t participate. We appreciate your time and kind consideration.
